This is another breakout of the Patient-Centered Medical Home functionality: Centralized Medical Records. This one is very obvious, but because patients are split between numerous referral networks, their medical records are also split. Please see the references on medical/mental inequities and disparities. This medical record splitting is likely a strategy used by everyone to get better care and a void the inequities/disparaties

Obvious – PCMH function breakout to support the learning associated with what a PCMH is.
